As a Social Worker in the Adult Care and Hospital Discharge Team, you will support adults with various needs transitioning from hospital to home or other care settings.
* Conduct comprehensive care assessments under the Care Act 2014 to evaluate individuals' physical, mental, and social needs.
* Assess mental capacity under the Mental Capacity Act 2005 for individuals unable to make decisions about their care.
* Coordinate hospital discharge plans, ensuring individuals have appropriate care arrangements.
* Monitor and review care plans post-discharge, ensuring services continue to meet individuals' evolving needs.
* Safeguard vulnerable adults, identifying risks of abuse or neglect and taking appropriate actions.
* Relevant social work qualification
