Job summary
Following successful recruitment of an Acute Frailty Clinical Specialist Occupational Therapist we are now looking for an Acute Frailty Occupational Therapist / Physiotherapist to support the development of our Acute Frailty service.
The successful candidate will work alongside an established Acute Frailty multidisciplinary team to support the assessment, care and discharge planning for patients attending 'The Front Door' presenting with a Frailty syndrome. Whilst predominately based within our Acute Frailty Unit and Frailty Same Day Emergency Care Unit, the role will include enhancing care for Frailty patients within other 'Front Door' clinical areas, including our Accident and Emergency Department, by working alongside the wider REACT team.
REACT's interdisciplinary way of working within Emergency Medicine's Assessment Areas and Short Stay wards aims to avoid hospital admissions, whilst providing high standards of care and discharge planning.
REACT are passionate about safely discharging patients back into the community and we achieve this through comprehensive holistic therapy assessments and working alongside external community partners.
Main duties of the job
As this is a newly devised post, the successful candidate will have the opportunity to develop their service improvement and leadership skills.
Applicants must be passionate about Frailty management, demonstrate relevant postgraduate learning and be ready to work within a fast-paced environment, for which a high level of problem solving and clinical reasoning will be required.
This role will be integrated into the REACT service which covers 7 days and extended hours, therefore working patterns will include long days and weekend working.
Job responsibilities
* Manage a caseload of patients with a varied range of conditions +/- complex needs using evidence-based practice with client centred principles in order to assess, plan, implement and evaluate interventions.
* Effectively identify and utilise appropriate discharge pathways and services in order to promote a home first approach and support patient flow.
* Provide specialist advice, teaching and training to other members of the MDT across the Emergency Medicine footprint regarding the identification and management of patients presenting with a frailty syndrome.
* Take a lead therapist role in managing clinical areas across the Emergency and Acute Medicine Footprint, providing day-to-day support to the wider team.
* To support the learning and development of junior staff and students.
* To participate in the planning, development and evaluation of the REACT service, including holding responsibility for defined projects specific to Frailty Care and Pathways.
* To participate within a 7 day working model, including extended hours.
