Job overview
The case management team is responsible for the case management coordination for patients with learning disabilities, mental health, physical and complex needs. Patient needs are met and regularly reviewed in line with national requirements for patients eligible for CHC, working within the Shropshire, Telford and Wrekin ICB.
As a Team Leader, Band 7, you will need to have a qualification as a registered nurse. We are looking for a person to lead a team of case managers in facilitating needs assessments, monitoring quality and assurance of care provision and financial agreements. In addition, you would be responsible for oversight of DoLs assessments and engage in complex case discussions to inform case management approaches.
Main duties of the job
You must possess a current NMC registration and have the ability to travel within the locality.
You will be expected to have a full understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, Who Pays guidance and relevant sections of the Mental Health Act and commissioning and contracting principles.
Ideally, you will be an experienced CHC assessor however, if you are an experienced case manager and do not have CHC experience we would still like to hear from you.
* The postholder will manage the coordination and monitoring of complex packages of care and NHS funded placement. It will be necessary to work closely with patients and their relatives ensuring that they understand the process, the eligibility criteria to be applied, and to provide empathy to patients.
* The postholder will have a flexible approach to care and will work across inter-agency boundaries, maintaining good communication and liaison skills with all professionals and members of the public.
* The postholder will undertake care reviews for people whose care is fully or partially funded by the NHS ensuring patient-focused support and facilitating the most appropriate and cost-effective pathway for individuals.

Detailed job description and main responsibilities
We are looking for experience of managing coordination of care provision and networking with other NHS professionals and local authority representatives to inform decision making. You will have learning and development opportunities to develop clinical knowledge of complex care, develop skills in negotiation and complex and sensitive communications and to be integral to making positive changes to patients’ lives.
* The postholder is accountable to the Head of All Age Continuing Care and Individual Commissioning and responsible to the Case Management Team Manager.
* The postholder will line manage locality case managers including workload allocation, monitoring, clinical supervision and the full range of line management duties, including appraisal, personal development and performance management.
* The postholder will be expected to work across the breadth of specialties according to their competency levels.
* The postholder will be responsible for the formal reviews and compliance with KPIs in relation to review completion for their locality case load.
* The postholder will take an active role and work as lead practitioner for a designated area/cohort of patients in facilitating and/or providing professional support, commissioning and case management of patients eligible for NHS Continuing Care.

Experience
* Significant experience of working in a clinical field and management of patients.
* Competent in case management.
* Line management experience within a clinical environment.
* Experience of CHC assessment.
* Commissioning experience.
* Previous experience in project management and planning.
* Skills for communication on complex matters and difficult situations.
* Ability to analyse and interpret information, pre-empt and evaluate issues, and recommend appropriate actions.
