The Integrated Discharge Team (IDT) is located within the Home First Directorate alongside teams of Occupational therapists, Adaptations, Short term reablement at home, Home emergency alarm response team, and Positive step, a bed based reablement unit.
The team works within the acute hospital at Doncaster Royal Infirmary and the emergency department 7 days per week identifying support required for people aged over 18 to return to their own home. This home first ethos is at the core of all the work we do. If this is not possible immediately from hospital, other pathways are considered for rehabilitation, continuing health care assessment, or further assessment in a bed-based setting, all with the aim of a return home.
Staff from the Discharge assessment and review team (DART) work in all these settings and with people in their own homes. Weekend working on a rotational basis is supported by all members of the team and takes place in Doncaster Royal Infirmary to learn or retain skills and knowledge in discharge assessments and offer flexibility across the wider team alongside team building with other staff in IDT.
The Discharge assessment and review team is part of IDT and works with people after their discharge, to assess and monitor long term care and support needs and safeguarding concerns for up to 6 weeks before longer-term work is undertaken by locality social care teams if required.
A strength-based approach is required to connect the person to their community and care and support when longer term needs are identified, engaging with other professionals in a timely way to achieve the outcomes that are right for the individual. Skills and knowledge in recording, report writing, and risk assessments are required for this role.
Applications are welcomed from registered social workers with relevant experience of assessment under the Care Act 2014, Mental Capacity Act 2005, GDPR, continuing health care, and safeguarding.
A Home first focus is used to support timely discharge of adult patients from the acute hospital. You will also work with carers to support, signpost and refer for assessment of their needs in accordance with the Care Act 2014.
As a valued member of the team, you will have access to peer support, 1:1 time and supervision from our Advanced Practitioners, the opportunity to support the development of the team and to work with our partners across the council, Social Care, Health, Housing and Voluntary Services.
This is a very fast-paced environment working to short timeframes and requires an organised approach and ability to work with several people and families concurrently.
