We have an exciting opportunity for a Band 5 Pharmacy Technician based on Osprey Ward.
Osprey Ward is located at Swallownest Court and is an 18 bedded mixed gender acute mental health ward. We work as a multi-disciplinary team to provide high quality care to patients experiencing a wide range of mental illness. We ensure our patients receive purposeful admissions, covering assessment, diagnosis, interventions, and treatment. We work collaboratively with a range of professionals including community mental health teams, social workers, and occupational therapists to support our patients to regain stability and facilitate a safe discharge into the community, ensuring patients receive effective post-discharge support.
Responsibilities
1. Admission
o Conduct a level 2 reconciliation and drug card check.
o Confirm and update allergy status.
o Take account of Mental Health Act consent to treat restrictions when administering medication.
o Assess patients' own medicines at admission for use within policy.
2. During stay
o Support the generation and adherence to patient level medicines care plans.
o Administer regular medication and prompt/support self-administration where appropriate.
o Administer PRN medicines, where appropriate, following nurse assessment.
o Have oversight of the adherence to patients’ monitoring with respect to medicine use i.e. GASS, ECG, physical health monitoring (within competencies).
o Co-ordinate leave medicines (to include label and supply within policy).
o Support MDT/PIPA meetings regarding medicines issues pertinent to the patient's care.
o Maintain the EPMA record, reporting gaps to ward management.
o Provide medicines information (verbal and/or written) to patients, involving pharmacist or other clinicians where necessary.
o Review and support the update and implementation of ward level medicines related SOPs.
o Respond to medicines related audit outcomes to ensure action plans are implemented.
