Weare looking for Health and Wellbeing Support Workers to work within our PrimaryCare Network, working alongside our, Clinicians, Mental Health Practitioners and CareCoordinators, to support both adults and children across our local population. You won't be on your own!

Main duties of the job
The Health and Wellbeing Support Worker willmanage and prioritise their own caseload, to help people take pro-activesteps to improve the way they manage their physical and mental healthconditions.
Usecoaching skills where appropriate to motivate individuals, their families andcarers, using a holistic approach, to identify their goals for care andactivation, with a possible case management approach;
Use motivationalinterviewing - work for change, space to explore barriers and try out newconcepts for managing emotional challenges and lifestyle issues. Encourage people to take personalresponsibility for their own wellbeing;
Develop, contribute to and participate in multi-disciplinary teammeetings across the PCN and with partners, to effectively manage casesparticularly where there is complexity;
Help and support people to access self-management education courses, peer support orother interventions that support them in improving their health and wellbeing;
Support delivery of personalisedcare plans by coordinating inputfrom a range of different professionals and services, and helping people andtheir carers/family to navigate across health and social care services;
Work with people over the phone, face to face,in the most appropriate place to provide support and in some instances carryout home visits.

North Kerrier East Primary Care Network (PCN) are part of the West Integrated Care Area in Cornwall, consisting of Leatside Health Centre and Veor Surgery, providing clinical services and health and wellbeing support services to people living in the towns of Redruth and Camborne, as well as surrounding villages. We have a strong focus on health promotion and personalised care, supporting people to make informed decisions about their care. We are a dynamic, innovative and inclusive PCN with experienced dedicated clinicial, non-clinical and management teams.

TheHealth and Wellbeing Support Worker is an integral part of the Health andWellbeing PCN team and is a key role in working with GPs; Mental HealthPractitioners, Care Coordinators and other practice and community teams tosupport practice patients.
The Health and Wellbeing Support Worker willmanage and prioritise their own caseload, to support people to take pro-activesteps to improve the way they manage their physical and mental healthconditions, based on what matters to them conversations, to help them improve the quality of their life, fosteringself-care, independence and choice.

Qualifications
Essential
6. Good level of education.
7. Enrolled in or undertaking or qualified in an appropriate health coaching/motivational interviewing training course.
8. A suitable and relevant qualification for working with adults and, or children.
Desirable
9. Higher level qualification such as NVQ in adult care Level 3 or qualification relevant to health or social care or a relevant qualification in working with children.
Experience
Essential
10. Experience of working in health, social care, education, third sector or other support services with direct contact with people, children, families, and professionals.
11. Experience of working with vulnerable adults and children.
12. Experience of working positively with people facing complex social and emotional challenges.
13. Experience in reflective listening skills and experience in non-clinical supported self-management.
14. Able to use a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving.
15. Ability to prioritise their own workload and meet deadlines.
16. Have an understanding of mental ill health and emotional distress.
17. Work with a diverse range of people from different cultural and social backgrounds.
18. Be able to work confidently and effectively in a varied and sometimes challenging environment.
Desirable
19. Experience of working in primary care and GP practices.
20. Experience or training in Mental Health Suicide Awareness for adults and children.
21. Understanding how to apply health coaching in group settings and working in groups.
22. Experience or training in personalised care and support planning.
