Job Overview We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will be responsible for managing client care plans, liaising with healthcare professionals, and providing support to clients and their families. Responsibilities Develop and maintain comprehensive care plans tailored to individual client needs. Supervise staff and ensure that all staff contribute to the efficient running of the service. Coordinate services between clients, families, and healthcare providers to ensure seamless care delivery. Monitor client progress and adjust care plans as necessary, ensuring that all parties are informed of any changes. Ensure that the care service is delivered to service users at the times and for the duration specified in the relevant individual agreement(s) and contract standards. Promote and protect the welfare of those individuals supported by the service. Maintain accurate records of client interactions, services provided, and outcomes achieved. Provide emotional support and guidance to clients and their families throughout the care process. Assist in scheduling appointments and follow-up visits for clients as needed. Collaborate with multidisciplinary teams to enhance the quality of care provided. Participate in training sessions and professional development opportunities to stay current with best practices in care coordination. Develop effective working relationships with other employees within the service. Work in cooperation with members of multidisciplinary teams to maximise opportunities for service users. Requirements Proven experience in a similar role or within a healthcare setting is preferred. Strong organisational skills with the ability to manage multiple tasks effectively. Excellent communication skills, both verbal and written, with a focus on empathy and understanding. Proficient in using office software and electronic health record systems. Ability to work independently as well as part of a team, demonstrating flexibility and adaptability. A genuine passion for helping others and improving their quality of life is essential.
