We are looking for a nurse with a background in acute and/or rehabilitation care and with a specialist interest and knowledge of stroke. Candidates need to demonstrate an enthusiasm and commitment to delivering and developing the Stroke service. The post-holder will provide specialist nursing practice to individuals who have had a stroke across both the acute and rehabilitation pathways of care.
The post holder will provide education and support to people who have suffered from a stroke as well as supporting other healthcare professionals, whilst being actively engaged in service improvement initiatives. You will be required to work autonomously but will be fully supported to develop within your role as a specialist nurse, while working closely with members of the Multi Disciplinary Team. This is an excellent opportunity for a Registered Nurse who is looking to consolidate their clinical skills while developing their own professional development.
Main duties of the job
1. To act as team leader taking responsibility for the organisation and day to day management of a team of nurses.
2. To effectively lead and be responsible for the ward team and clinical care carried out in a designated area in absence of and in conjunction with the Ward Manager.
3. Work alongside the Stroke Consultants and wider multidisciplinary team to provide expert care to patients across the stroke pathway.
4. The post holder will provide specialist educational support to a range of healthcare professionals, ensuring the most up to date stroke guidelines are followed.

Job description
Job responsibilities
1. Co-ordinate daily workload on apple tree ward, contribute for the 5 day week cover of stroke coordinator on site facilitating the stroke pathway. Deliver TIA and Stroke care based on principles that are research and evidence based and which enhance the quality of patient care.
2. Initiate subsequent referrals to the appropriate Multidisciplinary Team (MDT) and other appropriate referrals.
3. In the absence of the stroke ward manager, support the stroke unit nursing team to facilitate patient safety.
4. Daily contact with Hyperacute Stroke Units at Peterborough and Addenbrookes to ensure patient flow across the stroke pathway and timely/adequate repatriation of patients.
5. Assess patients with TIA/stroke liaising with stroke consultant facilitating admission to stroke unit.
