We are looking for a dedicated and compassionate Practice Nurse to join our healthcare team.
The ideal candidate will play a crucial role in delivering high-quality patient care, assisting with various clinical tasks, and ensuring that patients receive the support they need throughout their healthcare journey.
Main duties of the job
1. Long-Term Condition Management: Supporting patients with chronic illnesses such as diabetes, asthma, and hypertension.
2. Cytology (Smear Tests): Carrying out cervical screening in line with national guidelines.
3. Childhood Vaccinations: Administering immunisations as per NHS schedules.
4. General Practice Nurse Duties: Holistic care of patients.
About us
You would be working as part of a nursing team. You will be one of 3 nurses and 3 HCA, working as part of the bigger supportive practice team that consists of GPs, Pharmacist, Physio, MH worker, Social Prescribers, and our admin/reception team.
We currently have 11,500 patients and work from a purpose-built modern GP practice located in Blaydon town centre.
We are part of a PCN team and work closely with 3 other practices.
Job responsibilities
JOB PURPOSE: The implementation of Health Promotion and disease management strategies (in line with the National Service Frameworks) and provision of a full range of nursing services to the patients of the practice.
Chronic Disease Management as required by the practice.
The completion of health checks, health screening, immunisation and vaccination programmes.
MAIN DUTIES AND RESPONSIBILITIES:
1. Undertake immunisations, injections and vaccinations.
2. Provision of health checks.
3. Carry out screenings, e.g. blood pressure, cervical smears, etc.
4. Carry out chronic disease reviews e.g. diabetic, hypertension, asthma, CHD.
5. Undertake ECGs.
6. Provision of travel advice.
7. Venepuncture and the taking and preparing of pathology samples.
8. Provision of general nursing duties, e.g. wound care and compression bandaging.
9. Providing lifestyle advice and information for patients.
10. Management of medical supplies stock, including stock control.
11. The inputting of information onto the Practice clinical system.
12. Ensure that infection control procedures are followed.
13. The ongoing cleaning and sterilisation and recording of clinical equipment and working surfaces.
14. Working within PGD and PDS as appropriate.
15. Participating in nurse, practice and wider team meetings.
