Job summary Senior Health Care Support Worker - Holistic Virtual Ward Lincolnshire Community Health Service NHS Trust Job summary 2 WTE for North / North West Lincolnshire - Predominantly covering the Lincoln and Gainsborough areas but will be required to cover other areas of the county on occasion. This is an exciting opportunity to join the rapidly developing Holistic Virtual Ward which is a new service working with Lincolnshire Community Health Services, United Lincolnshire Health Trust and PCN's to develop an integrated workforce. As part of our commitment to ensure people with the most complex health and care needs are proactively supported to live well at home or as close to home as possible. The ideal candidate will be flexible and be able to work between the hours of 8am -6pm 7 days a week, with a view to 8pm finishing in the future. This will include bank holidays and weekends and will be with the supervision and support of Registered and Senior Registered Nurses and will involve driving long distances. Main duties of the job The Holistic Virtual Ward work in partnership with other agencies to provide holistic responsive and individualised support and assessment to patients across the county of Lincolnshire. Utilising a "What matters to me" approach that enables individuals to achieve their optimum physical, psychological and social wellbeing. Working as part of a multidisciplinary team contributing to improved health outcomes reducing unnecessary admissions to hospital and facilitating supported self-management. The role of Senior Health Care Support Worker will work across the County and community services to support provision of coordination to truly whole system integrated approaches to proactive care and support.
