Job summary We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills, who has the ability to work autonomously. The post holder will be an experienced nurse, acting within their professional boundaries to deliver evidence-based practice for patients with long-term conditions. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Main duties of the job Provide strong holistic leadership to the team and support the Advanced Nurse. To include day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture. The post holder will participate in and influence efforts across the health and social services to shape multidisciplinary pathways designed to support patient choice, improve quality of life, promote self- management and assure early intervention through the proactive provision of care. The post holder will provide an expertise in their own discipline to the wider team. Working within Professional guidelines and accountability, the post holder will have responsibility for the delivery of nursing care for the patients for a group of GP Practices as part of the Extended Primary Care Team. The post holder will have responsibility for the delivery of evidence based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population. Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes. The post holder will be required to work with a high degree of autonomy and professional accountability, demonstrating critical thinking and skills in clinical decision making in the management of their patients.
