Essex Partnership University Trust (EPUT) is currently looking to recruit HCA/Rehabilitation Assistants (Health Care Assistants in our rehab team) to join our community rehabilitation service. The Collaborative Care Team, the team works jointly with Frailty Virtual Ward and Community Stroke Team/Early Support Discharge Service. Main duties of the job The Collaborative Care Team is a multi-disciplinary therapy led team providing rapid assessment and rehabilitation for patients living within their own homes. The team support the facilitation of discharge and the prevention of admission for patients who require rehabilitation for a range of conditions including stroke, orthopaedics and unstable fracture management; neurological issues and jointly works with Frailty Virtual Ward team and service. As part of our friendly supportive team, you will be providing person centred care and supportive recovery. The service is provided across the whole of the South East Essex area including Castlepoint, Rochford and Southend and as such, it is essential that any applicant is able to travel efficiently and effectively across this area. The successful candidate will support people with personal care and assist in the provision of rehabilitation programmes during a range of shift patterns over seven days per week, in line with service needs.
