Job Title: Adult Social Worker – Hospital Team
Location: Thurrock
Rate: £28.05 per hour
Liquid Personnel is currently recruiting experienced and qualified Social Workers to join a busy team at Thurrock’s Hospital Team.
What will your responsibilities be?
The positions are required to facilitate hospital discharge and implementation of a new operating model that brings discharge functions into the community SW teams. There is also a need to work with colleagues and partners in a range of organisations to develop the appropriate service and support to meet the needs and outcomes of vulnerable residents. Additionally, you will ensure safeguarding procedures are adhered to and implemented accordingly.
