An exciting opportunity has arisen within the Warrington and Halton Learning Disability Community Team for an Advanced Practitioner to be a key senior member of the Intensive Support Function of the team.
The successful applicant will be working alongside the established Community Learning Disability Team to develop, initiate, lead and manage clinical assessment, formulation and interventions, staff training and special projects consistent with the aims of the Trust, service plan and service operational policy.
This post is subject to an enhanced check with adults barred listDisclosure and Barring Service (DBS) Check. Successful candidates will be required to pay for the cost of the DBS Disclosure which is £44.00, the cost of which will be deducted from first salary.
The post holder will be a key practitioner of the Enhanced Intensive Support Function of the Team (ISF), working alongside the established Community Learning Disabilities Team, Clinical Team Managers and associated specialists to develop, initiate, lead and manage clinical assessment, staff training and special projects consistent with the aims of the Trust, service plan and service operational policy.

CLINICAL:
Provide expert clinical practice to clients whom pose significant challenges/risk to themselves or others with associated problems e.g:
Challenging behaviour
Mental health issues
Complex communication needs
Profound and multiple disabilities (including unstable/complex epilepsy)
Long-term conditions
Forensic behaviour
As an autonomous independent practitioner assume continued responsibility for caseload and clinically support interventions to clients with complex challenging behaviour and/or forensic behaviours who may be at risk of placement breakdown and/or hospital admission.
To take the lead role in facilitating/co-ordinating access to primary and secondary health care services for people with learning disabilities. Developing/influencing and implementing changes at both local and trust wide level.
Provide clinical specialist practice in relation to service users with complex needs who may display forensic/risk behaviours, undertaking complex assessments, formulations and risk management plans in conjunction with the service user, their families/carers and other professionals.
Responsible for the assessment of need, care planning and care management of clients and carers in accordance with the Trust Effective Care Co-ordination policy within Mental Health Services.
Provide training when required to a wide range of people internal and external (health, social services, voluntary/private sector staff, clients and carers).
Contribute to the strategic development of services to meet the Valuing People agenda through effective facilitation and leadership.
Ensure links to professional leads are developed and maintained
Ensure the safe management of medicines and adhere to safe practice and principles of administration in accordance with the NMC code of professional conduct and Trust Medicines Code.
Work in a professional and collaborative manner with the multidisciplinary team.
Participate and promote positive intra-agency working.
Set standards of service user care based on sound research findings.
Where indicated and following completion of the relevant training the practitioner will act as a non-medical prescriber, at all times working within their sphere of clinical competence.
MANAGERIAL:
Support effective leadership to the designated team.
Formulate and implement local clinical and operational practices and procedures as appropriate.
Ensure effective deployment of staff and other resources within the Intensive Support Team to deliver the required service and agreed standards.
Participate in the recruitment and selection process in accordance with Trust policy.
Ensure effective induction of new staff in accordance with Trust policy.
Ensure the implementation, monitoring and updating of an operational policy and philosophy for the clinical team.
Ensure the principles of Clinical Governance are incorporated in service delivery by maintaining a high quality service based on agreed clinical standards.
Ensure adherence and implementation of Effective Care Coordination within their scope of responsibility.
Ensure that Trust policies and procedures are understood and adhered to by all staff within their scope of responsibility.
Support to develop local protocols involving team members to ensure a high standard of service delivery is achieved and maintained.
Support and ensure staff participation in the formulation of new ideas/suggestions to improve service user care and service delivery.
Ensure that the Managing Performance Policy is adhered to.
Ensure that the Managing Attendance Policy is adhered to.
Participate in the performance monitoring of services in accordance with national and local targets.
Provide statistical information on workforce and service user activity as required.
Assist in the maintenance and updating of the team’s equipment inventories and service schedules. Ensure regular servicing, maintenance, repair or replacement as required adhering to Health & Safety, COSHH and Medical Devices Agency regulations.
Ensure effective processes of communication are established, maintained and regularly evaluated within designated team.
Establish positive networks with partner agencies to support the development and delivery of Community LD Services.
Maintain and monitor a framework for risk management and Health and Safety that ensures timely and effective planning and appropriate actions.
Investigate complaints and incidents in accordance with Trust policy.
Ensure all staff are provided with the necessary clinical and management supervision regarding the co-ordination and delegation of duties, to ensure a comprehensive and consistent delivery of care is maintained.
Ensure that all staff are regularly updated and have knowledge of all emergency procedures.
