Service Care Education are looking for a SEND Key worker to work within the Staffordshire Council on a 3-month contract.
Location: Tamworth
Job Role/Responsibilities:
1. Co-ordinate, facilitate, and create the assessment of need and the development of an education, health & care plan that outcomes are in line with the relevant legislation and Code of Practice.
2. Act as a key worker for adults and children with special educational needs & disability, coordinating support from qualified professionals.
3. Build inclusive relationships with individuals with SEND and their families, respecting diversity and supporting choice and independence.
4. Analyse information to assess needs, strengths, risks, and potential, identifying and managing risks appropriately.
5. Work with professionals across agencies to fully understand needs and enable individuals to achieve their goals.
6. Explore innovative options with individuals and families to meet agreed outcomes.
7. Arrange suitable support packages, negotiate costs, and ensure families are informed of financial responsibilities.
8. Conduct regular case reviews and annual assessments, adapting plans as necessary.
9. Maintain detailed, professional records of assessments and decisions, ensuring compliance with SEND EHC policies.
10. Engage in quality assurance and personal development to enhance SEND EHC services.
Qualifications:
NVQ3/equivalent qualification (or a willingness to undertake) and experience in a relevant discipline (e.g., education, social care, childcare, advice and guidance).
Knowledge/Experience Required:
1. Significant experience in planning personalised support for individuals with disabilities.
2. Awareness of physical, intellectual, emotional, and social development across ages.
3. In-depth knowledge of disabilities and special educational needs (SEN).
4. Proven experience in collaborative teamwork.
5. Skilled in empowering individuals and families to plan for outcomes.
6. Proficient in using IT for documentation and reporting.
7. Knowledge of safeguarding and relevant legislation.
8. Understanding of the roles of partner organisations.
