Job Summary
We are seeking a dedicated and skilled Advanced Nurse Practitioner to join our healthcare team, working between our Bridge of Don and New Dyce Practices. The ideal candidate will possess a strong foundation in patient care, with a focus on delivering high-quality medical services. As an Advanced Nurse Practitioner, you will play a crucial role in assessing patient needs, developing treatment plans, and providing ongoing care to ensure optimal health outcomes.
Responsibilities
1. To undertake the role of an Advanced Nurse Practitioner, utilising high levels of decision making and clinical judgment to assess and treat a diverse range of paediatric and adult patients presenting in General Practice.
2. Ensure that patient health needs are met through the provision of the diagnosis and the management of patient conditions through the assessment, treatment and review planning process in partnership with patients and families.
3. Provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives. The ANP role encompasses that of expert clinician, educator, innovator and auditor.
4. To work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the development and delivery of policy and procedures, development and implementation of nursing services and providing nurse and clinical leadership as required.
5. Conduct comprehensive physical examinations and health assessments of patients.
6. Diagnose and manage acute and chronic illnesses while providing appropriate treatment plans.
7. Administer medications and monitor patient responses to therapies.
8. Maintain accurate and up-to-date patient records in compliance with regulatory standards.
9. Stay informed about advancements in medical practices and incorporate new knowledge into patient care.
Requirements
1. Registered Nurse. Educated at SCQF Level 11 (Msc) including advanced clinical decision making skills or equivalent and demonstrating case management appropriate to Primary Care.
2. Independent Supplementary Prescribing (v300) experience. In depth understanding of the management of Long term Conditions.
3. Evidence of continued professional development including paediatric and child health Evidence of management, education and training.
4. Effective listening and interpersonal skills.
5. Time management skills/ability to prioritise workload. Evidence of research/audit experience.
6. Evidence of effective problem solving skills.
7. Competent in standard IT packages e.g. Microsoft Word, Excel & TEAMs (A Digital and Data Capability Framework Assessment will be required as part of your application).
