A family support worker post to provide a holistic and integrated service for children and young people under the care of the Complications of Excess Weight (CEW) Service based at Bristol Royal Hospital for Children. Working alongside the CEW Multidisciplinary Team (MDT) including Paediatric Medical Consultants, Specialist Nurse, Psychologists, and Dietitian. Bristol CEW service is the hub site providing training and leadership to other CEW clinics in the Southwest.
It involves working with children, young people, and families, in order to facilitate lifestyle changes and improve health and wellbeing outcomes for children and young people with complications associated with excess weight.
The role requires assessment and delivery of care in the community as well as outpatient and inpatient hospital appointments involving the patient, parents, and family systems where there may be complex social needs. The post holder will be a member of the wider Psychological Health Services Department.
As part of the MDT, the family support worker will need to develop a therapeutic relationship with families to establish communication and trust to provide support for children and their families under the care of the team.
There will be direct and indirect work with children, families, carers, and other professionals so that meaningful and positive changes can be made to improve health and wellbeing. The post holder will be an advocate and facilitator to help to understand and resolve issues that may be perceived as barriers to care. Working alongside colleagues to offer emotional support, parenting, and signposting advice to patients and families.
An opportunity to develop assessment and intervention skills as well as contribute to a holistic approach to supporting families within a health setting. The post holder will be vital in supporting safeguarding practices alongside the MDT.
An opportunity to work autonomously and alongside an experienced team of clinicians both within clinical and community settings, the post holder will need empathic communication skills as well as being organised and able to prioritise workload demands. There will be many opportunities to collaborate with families, CEW MDT, and other professional or voluntary services supporting families referred to our service.
