Bradford and Airedale Community Nursing services are expanding its Leadership Team. Adult physical health Nursing delivers high quality to patients in their own home, clinic settings, and provides a variety of community nursing services.
We are looking for expert and aspiring leaders in health services to join our Community Team Leader group in our planned care community team in North Bradford localities.
The Community Team Leader position offers the opportunity to work with Operational Managers and Clinical Team leaders to deliver on our transformational plans to implement the NHS plan. This includes developing our Planned and Unplanned services as well as supporting a more integrated model of care for our Specialist services.

Main duties of the job
The post holder will provide leadership, and take managerial and supervisory responsibility to a group of clinical practitioners working as a multidisciplinary team providing adult physical health services [APH] in the community. Although this role will have a key focus on leadership and management within the team and developing the service, Clinical expertise is required to support staff in community nursing; therefore, community nursing experience would be an advantage.
The role is both rewarding and challenging where you will make new networks and gain additional knowledge and skills. You will work with planned and unplanned District Nursing teams, and in addition, you will work alongside other key members of the primary health care team, other health and social care professionals to provide proactive safe care to our local population.
