Our Team: Our Care Leavers Team are a caring, supportive unit, who are dedicated to improving the lives of our service users and providing them with an easy and effective transition to independent living. Your Contributions: As an Outreach Worker, you will assist in the provision of services to enhance the ability for all young people leaving care and so enable them to realise their full potential and become successful in all aspects of their lives. Some responsibilities include: Providing person-centered support to help young people develop independent living skills and integrate into their communities. Delivering housing support, including tenancy-ready courses, preventative work, and interventions. Facilitating the care leaver drop-in and hub, ensuring access to essential services and support. Building strong relationships with young people, advocating for their rights, wellbeing, and best interests. Maintaining records and working collaboratively with Personal Advisors, partner agencies, and other professionals to ensure effective service delivery.
