Wessex Children’s Hospice Trust (Naomi House)
This is an exciting new opportunity; the role will be right at the heart of shaping Naomi House and Jacksplace for the future. Working closely with the Director of Care (as Registered Manager), the successful Advanced Nurse Practitioner (ANP) will be the first point of contact for children and young adults with all types of health needs in-house, working as an autonomous practitioner and in conjunction with other professionals to make decisions and apply appropriate nursing interventions to the children and young adults. This will include history taking, clinical assessment, diagnosis, treatment, and evaluation of care; working within the Hospices and NMC policies, protocols, and guidelines, demonstrating safe clinical decision making, high levels of critical thinking and holistic practice. You will be expected to act independently but will have access to a Consultant/Associate Specialty Doctor for advice and additional support as necessary.
Main duties of the job
You will also be expected to be a senior clinical leader and expert within Naomi House and Jacksplace, and at times work alongside other care teams and professionals within hospital settings and the community.
As a qualified Advanced Nurse Practitioner, with experience working with children and young adults you must be able to demonstrate safe clinical decision-making, and sound clinical expertise and knowledge when working with children and young adults. The post holder will support the leadership team to meet the needs of the service users and contribute to service improvements within an environment undergoing change and service development. You will need drive and determination, excellent interpersonal and communication skills and a passion and vision that motivates others.

Job Summary

This will include history taking, clinical assessment, diagnosis, treatment, and evaluation of care. He/she will work within the Hospices and NMC policies, protocols, and guidelines, demonstrating safe clinical decision making, high levels of critical thinking and holistic practice. The post-holder will be expected to act independently but will have access to a Consultant/Associate Specialty Doctor for advice and additional support as necessary.
The ANP is expected to be a senior clinical leader and expert within Naomi House and Jacksplace and at times work alongside other care teams and professionals where indicated.
Key Responsibilities
1. Practice as an ANP within own sphere of competence and agreed acceptable limits of practice being accountable for own professional actions.
2. Be responsible for assessing, diagnosing, interpreting investigations, and treating the children and young adults within the In-House Hospice service within a developing scope of competence.
3. Work as part of the multi-disciplinary team in providing quality evidence-based health care to meet the immediate needs of the children and young adults.
4. Be responsible for contributing to the assessment of care needs and the development, implementation, and evaluation of programs of care.
5. Teach other clinical and non-clinical staff, supporting competency assessments and sign off.
6. Support day-to-day clinical leadership to the clinical team, participating fully in the delivery of healthcare provision.
7. Utilise clinical expertise and experiences towards developing and enhancing services within the Hospice and the wider health and social care economies.
8. Demonstrate the development and implementation of advanced skills and competencies and ensure that evidence-based practice is inherent in all aspects of care and treatment.
9. Ensure full and accurate documentation in the children and young adults notes of clinical history, physical examination, diagnosis and treatment plan.
10. Co-ordinate, and participate as required in any research projects and clinical audits.
11. A member of the Clinical On-Call rota, providing out of hours support for the hospice teams.

9. Ensure all resources, human, financial and environmental, are used in the most economic, efficient and equitable way.
10. Ensure that Health and Safety requirements are met and that risks to service users and staff are assessed and action taken to minimise these.
11. Participate in the development, monitoring and audit of policies and SOPs relating to clinical practice and care, and ensure that these are adhered to across all areas of work with children, young people and families.
12. Act as a representative of the organisation taking part in professional relationships, collaborative working and external promotional activities as appropriate encouraging partnership working.
Clinical Responsibilities
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for the children and young adults presenting with undifferentiated and undiagnosed conditions and complex needs.
2. Clinically examine and assess patient needs from a physiological and psychosocial perspective, and plan clinical care accordingly.
3. Proactively identify, diagnose and manage treatment plans for the children and young adults at risk of developing a long-term condition (as appropriate).
4. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan in liaison with the child, young adult and family as appropriate.
5. Ensure that where a child or young adult requires referral to a service for their health needs it is made by the same day.
6. Refer appropriately children and young adults to other healthcare professionals as required.
7. Prescribe and review medication for therapeutic effectiveness, appropriate to the children and young adults needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
8. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
9. Prioritise health problems and intervene appropriately to assist the children and young adults in complex, urgent or emergency situations, including initiation of effective emergency care.
10. Always working within your scope of professional practice, recognising your limitations and when to seek advice and support from peers. To ensure you provide the children and young adults with robust safety netting advice in relation to their health need providing a safe transfer.
11. Work supportively alongside clinical and administrative colleagues in ensuring the highest standards of care for all the children and young adults.
12. Maintain accurate and contemporaneous records.

* Experience of working as an ANP in a Ward environment.

Qualifications
* REGISTERED NURSE with valid up to date registration with NMC.
* RSCN/RN Child or Adult.

* Clinical Examination and Diagnostics Level 7.
* Eligibility to work as a Nurse in the UK and with UK current work permit if required.
* Ventilation trained.
