Lincolnshire's Community Children's Nursing (CCN) Service is expanding!!
Due to significant investment, we are looking for experienced children's nurses to join our Grantham CCN Team. If you are passionate about community nursing and want to be part of our exciting journey - we would love to hear from you! We would welcome applications from children's nurses currently working in ANY setting, e.g., hospital-based, health visitors, and school nurses.
Our CCN Team at Grantham is an integral part of our children's community services and consists of a Team Leader, Band 6 CCNs, and HCSW who work closely to provide a seamless service between hospital and home.
The service is available Monday to Friday, 9 am to 5 pm; however, team members are required to work flexibly to provide 'before' and 'after' school visits as required. You will participate in an evening telephone on-call system operating Monday to Friday from 5 pm to 10 pm on weekdays and 9 am to 10 pm at weekends and Bank holidays for the county-wide CCN caseload. This will run alongside a 24/7 rota for children and young people requiring end-of-life care when required.
Applicants must be NMC Registered Nurse (child). Recent CCN and community caseload management experience would be highly advantageous, as would a community health degree with a specialist practitioner qualification, although not essential as training will be provided.
Main duties of the job
Working as part of the team and in partnership with the child, family/carers, you will formulate individual care packages that facilitate the smooth transition from hospital to home.
You will have responsibility for managing your own caseload and coordinating services to facilitate early discharge, reduce hospital re-admissions, and provide support and nursing care to children and young people with acute healthcare needs and those with long-term conditions at home. Participating in the out-of-hours telephone on-call system to support all children and families on the CCN countywide caseload will enable admission avoidance.
As an excellent communicator, you will support the development of ambulatory care and promote the ethos of empowering children and their families to develop the knowledge, skills, and expertise to enable them to become self-caring, independent, and manage their own condition at home wherever possible.
Applicants must demonstrate an ability to work autonomously and as part of a team. The ability to travel throughout the county is essential. ULHT offers a lease car scheme for employees.
