Job summary The Hamilton Practice is seeking a motivated and approachable practice nurse, on a part time basis for 20 hours per week, over 3 days, although this is flexible. Informal visits are welcome. Main duties of the job Advise on childhood and adult vaccinations, ensuring safe administration. Run well-person clinics and health promotion programmes, providing holistic assessments and lifestyle advice. Promote healthy living and self-care principles. Perform holistic assessments for cervical cytology smear tests. Conduct venepuncture and take ECGs. Maintain infection control guidelines and manage stock and equipment. Assess, plan and evaluate treatment programmes for long-term conditions. Work with the MDT to manage long-term conditions using management plans. Support care for patients with asthma, COPD, diabetes, hypertension, cardiovascular disease, contraception, cervical cytology, immunisations, weight management, mental health and learning disabilities. Act as a Diabetes Specialist Nurse (DSN). Review patient needs holistically and provide person-centred care. Maintain accurate records and participate in clinical governance measures. Maintain effective relationships and communication with colleagues. Share learning and information with team members. Delegate tasks appropriately.
