Are you an experienced RMN looking to make a meaningful difference in dementia care? We are seeking a Registered Mental Health Nurse (Band 6) to join our client's multi-disciplinary Memory Assessment Service (MAS) team in Morecambe. This is a fantastic opportunity to work in a specialized role, supporting patients and their carers through high-quality assessment, planning, and care.
Key Responsibilities:
1. Serve as an independent specialist practitioner, ensuring effective assessment, planning, consultation, and formulation towards diagnosis within the Memory Assessment Service.
2. Demonstrate specialist knowledge and understanding of strategies and interventions for dementia treatment.
3. Proficiently refer patients to other agencies within the service or external services as necessary.
4. Confidently identify risk factors and take appropriate actions to mitigate them.
5. Facilitate clear and effective communication between GPs, LCFT services, and non-statutory services to uphold the highest standards of patient care.
6. Provide mentoring and clinical supervision to staff and trainees on placement.
7. Actively promote Trust values within the MAS service, aligning with NMC and HPC codes of conduct.
8. Support the team manager in providing leadership within the MAS team, ensuring robust assessment, formulation, planning, and monitoring of care for patients and their carers.
Pay Rates:
Days: £22 - £26 per hour
Nights/Saturdays: £30 - £34 per hour
Sundays/Bank Holidays: £38 - £44 per hour
