Practice Summary
The West Park Surgery has an Outstanding CQC rating at the Long Eaton Health Centre site and at the branch site of Park View Medical Centre. We are a secure, successful and forward looking practice. The GP Partners hold key leadership positions in the PCN and the ICS and can provide a wealth of opportunity and experience to other members of the team. The practice is heavily involved in supporting and developing PCN services across the whole of Erewash.
We are part of Erewash Health Partnership and have access to a wide range of networked services including home visiting, care home support team, pharmacists, social prescribers and first contact physios. We offer all core services as well as minor surgery and contraceptive services.
Education remains an important part of the ethos of the practice, and we train registrars, medical students, non-medical prescribers as well as supporting apprentices from the local community. This culture of continuous improvement and development is core to who we are. Regular clinical meetings across both sites, network learning events and an allocated mentor mean that we create a safe and supportive environment.
Job Responsibilities
1. In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
2. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
3. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
4. Screening patients for disease risk factors and early signs of illness.
5. Developing care plans for health in consultation with patients and in line with current practice disease management protocols.
6. Providing counselling and health education.
7. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
8. Recording clear and contemporaneous consultation notes to agreed standards.
9. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
10. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
11. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
12. Awareness of and compliance with all relevant practice policies / guidelines, e.g. prescribing, confidentiality, data protection, health and safety.
13. A commitment to life-long learning and audit to ensure evidence-based best practice.
14. Contributing to evaluation / audit and clinical standard setting within the organisation.
15. Contributing to the development of computer-based patient records.
16. Contributing to the summarising of patient records and read-coding patient data.
17. Attending training and events organised by the practice, PCN or other agencies, where appropriate.
Person Specification
Knowledge
1. Knowledge of GP contracts/enhanced services/QOF.
