Ranworth Primary Care Network have an exciting opportunity for a qualified clinical pharmacist to join our PCN on a full time, permanent basis, Monday to Friday. This role is based at Kennedy Way Medical Centre.
The post-holder will work in the proactive ambulatory team, providing expertise in clinical medicines management, medication reviews and repeat prescription authorisation.
Main duties of the job
* Review the ongoing need for medicine of patients facing long-term conditions, monitoring needs and opportunities to support patients with their medicines ensuring they get the best use of their medicines (i.e. medicines optimisation).
* Undertake patient facing care home clinical medication reviews.
* Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signpost to community pharmacy and refer to GPs or other healthcare professionals where appropriate.
* Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
* Reconcile medicines following discharge from hospitals, intermediate care and into care homes.
* Identify and rectify unexplained changes and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
* Provide a telephone help line for patients with questions, queries and concerns about their medicines.
* Manage the repeat prescribing reauthorisation process.

Person Specification
Qualifications
* GPhC registered pharmacist.
* Minor ailments certificate.
* CPPE pathway completion preferred.
* MUR and repeat dispensing certification.
Experience
* 2 years + working as a pharmacist (or equivalent).
* Experience in managing pharmacy services in primary care.
* Broad knowledge of general practice.
