Job Title: Social Worker – Adults Disabilities Team 25+
Location: Conwy County Borough Council
Rate: £36 per hour
The Conwy County Borough Council currently has a short term vacancy for a Social Worker to work in our Disabilities 25+ Services.
What will your responsibilities be?
In this role, you will deliver a high-quality social work service to adults who are deemed vulnerable or needing support due to an impairment that is either physical or mental, and the impairment has a substantial long-term effect on the individual’s ability to carry out day-to-day activities. You will need to work with the citizen, carers, and families to provide support and to develop resilience and independence, ensuring that the citizen, families, and carers are at the centre of decision-making about their lives and the outcomes that they want to achieve.

Qualifications and Experiences:
To be successful in this role you must have Social Care Wales registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have minimum 3 years post qualified experience plus technology & software skills.
