We are looking to appoint a Lead Complex Case Advisor post working across the Health Board hosted in CHC Corporate services. Establishing if and how exceptional, complex patients, for whom existing care pathways are absent to meet their care needs, can be best supported by the Health Board. This will involve establishing common themes and transferable practical and governance supports through a quality and values approach making a positive difference to the patient’s experience. As Lead Complex Case Advisor you will ensure that all advice and in-reach support is professionally delivered in line with agreed performance, financial and government objectives. You will develop and lead a small team of complex care advisors to ensure that recurring themes and truly bespoke care pathway patients’ clinical-responsible teams have support and care pathway resolutions for the best patient experience and public value. Main duties of the job C andidates to demonstrate strong operational management, strategic and transformation skills. Have a positive ‘can do’ attitude, complex case expert possess an in-depth understanding of service functionality, transformation, improvement, governance, decision authorities and clinical standards. Able to demonstrate a passion for excellence in transforming quality outcomes, experience and access to care pathways for patients which will improve quality, efficacy and flexibility of bespoke complex case management working across clinical and organisational boundaries. An inspirational leader with a high level of complex case management credibility and ability to support expansion and development of a local framework and development plan for clinical colleagues and partners across the integrated care system. Provide leadership and in-reach advisory caseload support and guidance for the MDT. Ability to identify and work collaboratively with a wide range of stakeholders. Have a proven track record of transformational change management and demonstrate excellent leadership skills with an ability to develop the infrastructure, in a managed way, to support potential additional transformational roles within the team.
