An exciting opportunity has arisen for a Band 5 Registered Nurse to join our team on Ward C6. Ward C6 is a 21 bed Elderly medical ward under the care group community frailty, and therapy care group. We provide care for patients with both acute and chronic medical conditions as well as general medical and older people's care.
We are looking for dynamic individuals who are able to provide high standards of care, ensuring quality and best practice standards. All candidates are expected to be able to manage care for a group of patients, ensuring care priorities and workload is effectively managed. The candidate must display professional values in line with the nursing profession as well as uphold the vision and values of our ward area.
We are looking for someone who is dynamic and passionate in caring for and managing patients with often complex medical conditions and comorbidities. Candidates should be keen to develop knowledge and broaden their clinical competencies in areas such as managing patients with often multiple long term health conditions along with acute conditions requiring medical intervention and hospital stay. We are a highly skilled team and the candidate must be dedicated to continual professional development within our specialty. We work closely with physiotherapy, Macmillan Palliative Care Service and other Clinical Specialists.
Main duties of the job
1. Assessing patient care needs, planning, delivery and evaluation of nursing care, ensuring a patient centered flexible approach, working within best practice as well as local policy and procedure.
2. Ensuring priorities and care delivery are managed.
3. Ensure safe delivery of prescribed medications and monitor effectiveness, reporting as necessary to medical team.
4. Effective monitoring of the patient's clinical condition using tools as well as clinical judgement. Escalation as required.
5. Appropriate harm reduction measures to promote safety for the patient.
6. Work effectively within the team and MDT, advocating for the patient as needed.
7. Safe admission, transfer, discharge and handover of care of the patient.
8. Communicate effectively with the patient and those important to them, ensuring they are well informed and kept up to date, ensuring the patient's wishes and priorities are respected.
9. Knowledge of legal frameworks with regards to consent & capacity, safeguarding adults and vulnerabilities.
10. Demonstrate clinical and professional knowledge. Dedication to mandatory training as well as courses/CPD that will enhance knowledge and competence.
11. Clear and concise record keeping.
12. To be able to undertake the role of the shift leader once trained and competent, to ensure safety and oversight of the clinical area in absence of the ward manager or deputy.
13. To supervise other qualified and unqualified staff and contribute effectively and positively to their learning.

Person Specification
Education and Qualifications
* Registered Nurse - Adult - Current NMC UK Registration
* Experience of working as a Registered Nurse in a Hospital Ward Environment
* Practice Supervisor/Assessor
* Evidence of post registration training such i.e. CVADD, NG Tubes, Catheterisation, cannulation, Venepuncture (certificates must be available upon request)
Occupational and Knowledge
* Ability to monitor closely and provide clinically effective care for a group of patients with varying conditions and care needs.
* Uphold Professional Nursing Values and Visions.
