Job summary
The Discharge Lounge is offering an exciting opportunity to join the team as a permanent Band 5 Registered Nurse.
The successful candidate will have experience of acute hospital care and a knowledge of patient discharge processes.
The role requires the individual to have enthusiasm and a passion to drive the service forward and meet service demands.
They should have effective communication skills and an ability to support all the staff within the team to deliver high quality safe patient care.
The Discharge Lounge supports all areas within the Trust with an emphasis on maintaining timely and safe patient flow out of the Hospital.
Working Pattern: 37.5 hours per week during the hours of 07.30 - 20.00 Monday to Friday and 10.00 - 16.00 weekends.
Main duties, tasks & skills required
1. To assist and facilitate Patient Flow for the wards and patients in a timely discharge process.
2. To provide clinical leadership within the ward area.
3. To provide and maintain a high standard of evidence based, patient centred care.
4. To demonstrate good interpersonal skills and to develop practical skills required within the specialty, whilst reflecting the philosophy of care.
5. To deliver patient care daily, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.
6. To provide direction, supervision and control to team members in order that they provide patient care and services which are appropriate to the individual needs of the patient and the ward/unit.
7. To provide a friendly, effective reception, service to patients.
8. Support and assist Discharge Lounge staff in the day-to-day management and contribute to the care environment.
9. All staff are expected to work to the Trust Values of Outstanding Care, Working Together, Leading and Listening.
10. The duties listed are not exhaustive and may be varied from time to time.
11. The post holder will be expected to undertake other duties as appropriate and as requested by his/her manager.

Qualifications
Essential
Nursing, professional registration.
Experience
Essential
* Good understanding of nursing care methods and models.
* Previous experience of working within an NHS environment.
* IV Drug Administration.
* Able to identify priorities.
* Innovative, able to problem solve and make decisions.
* Understand the importance and relevance of nursing research and uses this to improve clinical practice.
* Able to use resources efficiently and effectively.
Desirable
* Established organisational and leadership skills.
* Teaching skills.
Additional Criteria

* Typing skills - to use computerised systems for accessing data, interpretation of data, patient information, developing a data base of staff training records and typing of correspondence and meeting minutes.
* Able to demonstrate a good standard of English language.
* Able to communicate verbally and electronically to a high standard.
* Highly effective communication skills - able to communicate verbally and written to staff, patients and relatives including in situations of conflict & distress, ensuring that communication is tailored to the person being addressed.
* Able to document observations, results, decisions, and actions etc. effectively in patient records and communicate these effectively to appropriate members of the multidisciplinary team.
* Good organisational skills.
* Able to work under instruction and under own initiative within the boundaries of the role and as part of a team.
