We are a GP Practice with a multi-skilled clinical team that works over four sites across Bury (in Bury town centre, Greenmount, Tottington and in Radcliffe).
We are looking for an experienced Practice Pharmacist to work as part of our multi-disciplinary team, supporting our focus on quality, safe and cost-effective prescribing right across the Practice.
As a Practice Pharmacist you will implement, use, and review systems and processes to ensure evidence-based prescribing, effective medicines management and continual improvements.
You will be experienced in all areas of prescribing and medicines management, providing specialist advice, and training to our team, whilst supporting our patients through medicine reviews and monitoring.

Main duties of the job
1. Support the development of high quality, safe and cost-effective prescribing in the practice, utilising and reviewing prescribing systems and implementing processes to ensure evidence-based practice is embedded in patient care.
2. Support the partners in implementing effective medicine management within the practice, identifying areas for improvement, and initiating and managing change.
3. Develop and manage a medicines management plan and deliver patient services as determined by practice policy and local and national guidance.
4. Develop the medicines team, including delivery of training, in order to maximise cost-effective prescribing and improve the quality of patient care.

We are a large GP Practice with a multi-skilled clinical team that works over four sites. Our sites are located in Bury town centre, Greenmount, Tottington and in Radcliffe.
We provide services to around 50,000 registered patients, that’s a quarter of the population of Bury, and our patients can access services across all four of our sites.
Job responsibilities
1. To provide clinical leadership, in relation to medicines management, and, as a senior clinician, contribute to the development of primary care services for the population served.
2. To provide specialist pharmaceutical advice, on a day-to-day basis, in all aspects of prescribing and medicines management to GPs, nurses, other practice staff and patients as appropriate.
3. To monitor practice prescribing costs, and local and national prescribing targets, reviewing prescribing patterns, financial changes, and adherence to local guidelines and formularies.
4. To develop practice specific protocols and formularies in line with agreed CCG and National guidance, to support high quality, safe, cost-effective, and evidence-based prescribing.
5. Oversee the repeat prescribing reauthorisation process ensuring that it is safe and robust for all patients. Ensure that there is a safe review process for patients on high risk drugs and for patients at risk of admission.
6. To undertake clinical medication review and support the management of long-term conditions and self-care within domiciliary or community settings, as appropriate. Reducing inappropriate polypharmacy and wasteful prescribing.
7. To support the implementation of CCG initiatives around long-term conditions and self-care, providing health promotion and preventative healthcare advice when necessary and liaising with other healthcare professionals as appropriate e.g. secondary care providers, chronic disease management team, and integrated care team.
8. Ensure that medicines are reconciled in a timely manner and work with patients, GPs, carers and care providers and other health professionals to ensure patients receive the medicines they need in a timely manner.
9. To provide phone advice regarding medicine-related queries and minor ailments and be able to triage appropriately.
10. Working within professional boundaries and competencies, provide face-to-face clinical advice in relation to medicine-related queries and minor ailments.
11. To maintain clear, accurate and contemporaneous computerised records of all patient contacts.
12. To work with prescribers and GP trainees to undertake clinical audit as appropriate, within the practice to demonstrate NICE implementation, as part of CCG work plan or as a result of identified clinical issues within the practice.
13. To assist in the education and training of healthcare professionals employed within the practice, in relation to medicines and the prescribing system.
14. To assist the practice in managing the introduction of new drugs, and drugs that have been withdrawn.
15. Co-ordinate the response to medicine-related alerts received from the MHRA and Drug companies according to agreed local policy and guidance.
16. To work with local pharmacies to address drug supply shortages causing minimum disruption to patient care.
17. To support the implementation of National medicines management initiatives e.g. Repeat Dispensing Scheme and Electronic Prescribing.
Person Specification
Skills
* Therapeutics in-depth knowledge of drugs in current use.
* Awareness of the breadth of common acute and long-term conditions that are likely to be seen in general medical practice.
* Experience of using Word, Excel, PowerPoint and Outlook.
* Drug information/critical appraisal skills.
* Ability to interrogate and extract accurate information from GP clinical systems.
Qualifications
* M.Pharm or BSc/B.Pharm (prior to 2001).
* Registration with the General Pharmaceutical Council.
* Supplementary prescriber / Independent prescriber.
* Member of the Royal Pharmaceutical Society of Great Britain.
* Working towards RPS Faculty membership.
Knowledge
* Knowledge and experience of clinical audit.
* Working across boundaries good communication skills, able to collaborate and co-operate in multi-agency working.
* Familiarity with General Practice prescribing and factors affecting prescribing.
Experience
* Eligible to work in the UK.
* A minimum of 3 years post-registration experience.
* Experience of working within a healthcare team environment.
* Previous experience of working with a range of healthcare professionals.
* Familiarity with Prescribing Analysis and Cost data.
* Understanding of Primary Care systems and structures.
* An appreciation of the nature of GPs and General Practice.
* Experience of data collection, analysis and formulating reports.
