We have been successful in securing permanent funding to support Discharge to Assess pathways in North East Essex. This exciting development enables us to fully integrate a range of options to support discharge plans & ongoing rehabilitation from hospital, working closely with hospital teams, reablement and homecare.
We are looking for an experienced, motivated clinician to join our team. You will be a registered Physiotherapist with demonstrable expertise in goal focused rehabilitation, supporting patients to become experts in their own care & to remain in their own homes.

You should ideally have experience of working in a variety of clinical settings with knowledge of the challenges of community working.
