Referral & Assessment Manager - South Wales
To manage, co-ordinate and monitor all informal and formal referrals and expression of interest for services within allocated region/division.
You will be employed on a hybrid basis and work in a specific region/division; however, you may also be expected to support in other regions/divisions when required.
Main duties of the job
* Monitoring current service vacancies and being aware of potential vacancies
* Develop a pipeline of referral approaches that promote our core services
* Monitor & respond to referrals from various external sites and through direct contact
* Have access to relevant and up-to-date marketing packs for each service
* Maintain a current tracking system to enable clarity of referrals
* Produce regular and timely reports on referral activity
* Co-ordinate internal referral meetings to ensure that the service team is best placed to welcome any new referral
* Lead on assessments for new referrals, working alongside the service
* Complete and submit the needs assessment, with relevant support package which includes costings
* Provide transition support when a new referral is confirmed to ensure that the service is able to deliver a smooth transition
* Build effective relationships with commissioners, stakeholders, and attendance at provider forums
* Seek new opportunities to grow services including analysing referral data to understand any localised commissioning needs
* Enable a solution-focused approach to referrals and transition to ensure all barriers to delivering new support are removed
* Ensure that contract and agreement documentation is present for new referrals

Person Specification
Personal Qualities
* Ability to develop services.
* Experience of working with different agencies.
* Professional registration.
Experience
* Management experience gained in the social care sector including assessing individuals.
* Management of relationships and networking to promote services.
* Experience of working with LD, ASD conditions.
* RM experience.
* Experience of delivering learning/training.
Qualifications
* Evidence of CPD.
* Management or training qualification.
