Position: Support Worker
Responsible to: Case Manager, Registered Manager
Remuneration: £11.70ph paid weekly

Purpose
The purpose of the Support Worker role is to assist our service users to remain as independent as possible in their own homes and to achieve their goals, which are personal to them. Our Support Workers deliver care in the home and other venues and work to the bespoke support plans which have been fully designed with our service users.
Duties and Responsibilities
1. Deliver high-quality care to service users in line with the values and promises of Sphere Specialist Healthcare.
2. Encourage service users to remain as independent as possible.
3. Be aware of the personal needs and preferences of your service users.
4. Communicate appropriately and sensitively with service users using various methods depending on their needs.
5. Promote dignity, respect, and privacy of service users and their families.
6. Support the families and friends of service users during the care process.
7. Liaise with the Multi-Disciplinary Team involved in the care of the service user.
8. Deliver care sensitive to the service user’s culture, disability, race, age, sexuality, marital status, gender identity, or religion/belief.
9. Support service users in integrating into and contributing to their community.
10. Assist service users with mobility problems and the use of personal equipment.
11. Promote mental and physical well-being through engagement in activities.
12. Support service users in achieving their care plan goals.
13. Read and write reports and participate in meetings and training activities.
14. Comply with the organisation’s policies and procedures.
15. Report significant changes in the health or circumstances of a service user to the registered manager.
16. Attend mandatory training, including induction and relevant qualifications.
17. Support service users in maintaining autonomy and choice.
18. Assist service users in identifying and mitigating risks.
19. Support service users with capacity issues appropriately.
20. Consult with service users and their families to ensure their views are acted upon.
21. Engage in continuous professional development.
22. Recognise and report practice that does not meet defined standards.
23. Contribute to regulatory body inspections or monitoring visits.
24. Safeguard service users from abuse and maintain confidentiality.
25. Perform other tasks as requested by line managers.
