Community Nurse - Chirk / Overton District Nursing Team

We currently have a vacancy for a permanent Band 5 Community Nurse post working 32 hours per week within the South Wrexham District Nursing Service based at Chirk and Overton. You will be required to work across both District Nursing Team bases.
The post requires that the successful candidate is a registered nurse with evidence of continued professional development. They should be motivated and enthusiastic, work within the team, whilst having the ability to work independently. The successful applicant will require an awareness of the evolving role within District Nursing.
Main duties of the job
Our aim is to provide quality, planned care for the patients. The post holder will be responsible for assessing, planning, implementing and evaluating care in the community. The post holder will work as part of the district nursing team and with other agencies to provide clinically effective care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications
* NMC Registered
* Evidence of post registration study and training, e.g. POVA, Child Protection, Intermediate Life Support
Experience
* Experience of providing holistic nursing care
* Clear understanding of clinical governance framework
* Implement evidence based practice
* Clear understanding of contract of care
* Ability to document details clearly and accurately
* Clear understanding of consent process
* Basic IT skills
* Knowledge of incident reporting policy
* Awareness of Policies Knowledge of incident reporting policy
Personal / Disposition
* Ability to communicate effectively
* Ability to work under pressure.
* Welsh Speaking
