Who You Are The role is for an experienced social worker with a minimum of 2 years' experience, who will work as part of the Lewisham and Greenwich NHS Trust’s community rapid response service. You must hold a degree in social work, be registered with a professional body, and be committed to continuous professional development. You will work four days in the office and one day from home, primarily intending to prevent hospital admissions by treating patients at home. What the Job Involves You will provide social work support to the UCR team. Your responsibilities include conducting timely social care assessments, offering crisis intervention, urgent social care support, collaborating with reablement services, providing urgent support during carer breakdowns, and more. You will work closely with local authorities, healthcare professionals, care providers, and other relevant services to ensure comprehensive care. Regular liaison with community resources and voluntary organizations is also required to arrange additional support where needed.
