We are looking to appoint a Physiotherapist to join the Intermediate Care team which is part of the Neighbourhood Rehabilitation Service (NRS) working across Barnsley Community.
The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community therapy assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission.
You will assess, manage & provide treatment to individuals with complex and/or multiple needs, working closely with the MDT providing supervision/support to other staff.
The team is committed to personal development & provides allocated time for CPD, training & wellbeing.
A vehicle to undertake travel is essential & the ability to work flexibly including evenings, weekends & bank holidays.

Main duties of the job
As Physiotherapist you will:
1. Manage a clinical caseload with autonomy.
2. Demonstrate knowledge, experience & relevant clinical skills for patients with complex/long term conditions, falls, orthopaedics & general mobility problems.
3. Undertake comprehensive assessment, provide therapeutic diagnosis using investigative, advanced analytical skills, specialist listening, physical & observational skills, interpret clinical findings/documentation & implement agreed treatment plans.
4. Monitor & evaluate treatment to measure progress/outcomes to ensure effectiveness of interventions & service delivery.
5. Develop programmes of care to support discharge.
6. Contribute to clinical leadership/supervision facilitating a culture of learning/reflective practice.
7. Have high level communication & excellent interpersonal skills.
8. Compliance with clinical governance being responsible for ensuring Infection Control policy/procedures are followed.

JOB SUMMARY

KEY RESULT AREAS
1. Job Purpose: To work as an integral member of the Physiotherapy and Clinical Teams within the service.
2. To carry out a specialised clinical caseload undertaking specialist assessment, treatment and advice to patients often with complex presentations and formulate a clinical diagnosis and physiotherapy treatment plan as an autonomous practitioner.
3. To be responsible for specialist advice to staff, patients and carers.
2. Job Dimensions: Supervision of students, assistant, and band 5 physiotherapist. No direct budget responsibility.
Person Specification
Experience
* Experience of risk assessment/moving and handling issues including the safe and correct use of all specialist equipment.
* Experience of integrated/multi-disciplinary team working.
* Experience in specialist clinical area directly related to the post.
* Experience of supervision of assistants/students.
Training
* Substantial demonstrable post registration rotational experience across several service areas to include respiratory care/medicine, neurology and older peoples service.
* Evidence of continuing learning & development through CPD activities/folder.
* Involvement in audit/research/quality issues or training.
* Post Graduate study/course in relevant clinical specialism.
Personal Attributes
* Must be able to work effectively to deadlines and time constraints.
* Must be flexible around working times and practices including evening and weekend work where necessary.
* To be able to travel around the Trust as appropriate to the role including transport of equipment.
Special Knowledge/Skills
* Specialist level of physiotherapy clinical reasoning, moving and handling, treatment skills and techniques related to the service area.
* Good communications skills verbal, non verbal and written.
* Ability to work in isolation, as part of a team, and on own initiative.
* Good organisational, prioritising, negotiation and delegating skills.
* Ability to teach and supervise.
* Ability to keep appropriate, accurate, legible patient records and data on paper and electronically, and awareness of confidentiality.
Qualifications
* BSc in Physiotherapy level 2:2 or above or equivalent. (Diploma).
* HCPC Registration.

* Ability to undertake the duties and demands of the post. A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and/or pregnancy).
