Hospice Certified Nursing Assistants (Healthcare)
Traditions Home Health Services is currently seeking compassionate, skilled, reliable, and experienced Hospice trained Certified Nursing Assistants to assist with our client who is currently on Hospice and resides in the Plymouth area.
Shifts available:
1. Morning: 3-4-hour shifts (typically 8a-12p, 9a-1p)
2. Day: 6-12-hour shifts
3. Overnight: 12-hour shifts (typically 7p-7a, 8p-8a)
CNA Job Responsibilities:
1. Assisting with daily living activities, including toileting, incontinence care, bathing, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance. CNA would document care and findings.
2. Monitoring vital signs, facilitating daily exercises according to the client's care plan, and performing range of motion exercises.
3. Preparing healthy, fresh meals in accordance with the client's care plan and ensuring adequate fluid intake throughout the shift and documenting client's intake.
4. Adhering to infection control protocols.
5. Monitoring client's skin integrity, doing skin checks daily and documenting findings.
6. Administering medication reminders and assisting with the application of eye drops, ointments, creams, and ear drops.
7. Continuously monitoring/assisting client for all transfers and ambulation as client is a fall risk.
8. Conducting housekeeping tasks such as laundry, folding, putting away clothes, changing sheets, vacuuming, washing dishes, and dusting to maintain a safe and clean home environment for the client.
9. Engaging with your client in fun, stimulating activities to create joyful moments.
10. Providing transportation for outings, errands, appointments, if required.
11. Skilled in assisting with transfers, operating mechanical lifts.
12. Proficient in dementia care.
13. Proficient in hospice care.
14. Competently documenting shift activities digitally hourly using a smartphone.
15. Reporting any concerns to the Traditions Home Health Services Office Support Team in a timely manner.
16. Working with the Hospice agency and treating staff with respect and as colleagues.
