The Harrogate Integrated Community Team have an exciting opportunity for a Community Support Worker. The successful candidate will support the team who work with working age adults with mental health needs. The team provides an assessment and a range of evidenced based interventions to improve the lives and outcomes of service users and their families.
The successful candidate must have completed the Diploma in Health and Social Care Level 2 or equivalent. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).
Main duties of the job
Undertake delegated activities within level of own competency and under the supervision of qualified staff, with due regard to risk assessment.
Supporting the delivery of interventions to people. graded exposure, anxiety management etc
Carrying out physical health monitoring such as blood pressure, temperature, pulse, be able to obtain blood and physical heath checks and discuss results with appropriate professionals within the team. This would also include ECG's, any follow up appointments required for further testing.
Supporting the development and implementation of care plans, providing appropriate feedback to clinicians and communicating outcomes appropriately.
