You will be working in a well established and skilled Occupational Therapy Team within the Specialist Community Learning Disability Service (SCLDS) at Sheffield Health and Social Care Trust.
We are a supportive and friendly team and you will have lots of opportunities for continued professional development with regular supervision.
You will hold an independent caseload of adult service users with Learning Disabilities and associated needs. You will also have the opportunity to be involved in multidisciplinary pathways such as Dementia, Autism and Eligibility.
You'll work closely with an MDT including Nursing, SLT, Physio, Music and Art therapy, and Psychology.
Main duties of the job
1. To manage a defined caseload, using evidence based/client centred principles to assess, plan, implement and evaluate interventions in community settings and on home visits.
2. To maintain accurate clinical records.
3. To develop skills and knowledge through participation in supervision, preceptorship, and PDR.
4. To contribute to the development and maintenance of the highest professional standards of practice, through active participation in internal and external CPD training and development programs.
5. To participate in multidisciplinary pathways which may include Autism, Dementia, and Eligibility.

Job responsibilities
1. To select and apply Occupational Therapy Assessments for a caseload addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity, leisure, and engagement.
2. To plan and implement client-centred individual and/or group interventions, using clinical reasoning and a wide range of treatment interventions to achieve therapeutic goals.
3. To monitor, evaluate and modify treatment for clients with multiple needs in order to measure progress and ensure effectiveness of intervention.
4. To complete outcome measures.
5. To provide OT advice and training to carers, clients, and other professionals.
6. To be responsible for the supervision and written assessment of Occupational Therapy students on practice placement within the Trust on completion of competencies.
7. To participate in the operational planning and implementation of policy and service development within the team.
8. To maintain a high level of knowledge of resources, service, and national issues relating to people who have a learning disability.
9. To apply specialist skills and knowledge in order to establish professional competence and fitness to practise as an Occupational Therapist.
10. To be actively involved in the development and implementation of the new service structure.
Work Schedule
The LD Service operates from 8am to 8pm and staff will be expected to work flexibly to meet the needs of the service and be able to travel independently across two bases: Firshill Rise and Grenoside Grange.

* Degree in Occupational Therapy.

* Commitment to client-centred, non-discriminatory practice.
* Knowledge of the importance of people's rights and acknowledgement of people's expressed beliefs, preferences, and choices, valuing people, taking into account own behaviour and its effects on others.

* Experience of working with people with Learning Disabilities.
Knowledge
Essential
* Ability to assess health and well-being needs as an Occupational Therapist in relation to the post applied for.
* Ability to develop, monitor, and review care/treatment plans to meet assessed needs in relation to the post applied for.
