Trelya aims to break the cycle of social exclusion, enabling adults and young people at risk of social exclusion due to social and economic deprivation to develop the skills, knowledge, awareness, aspirations, and motivation to make positive choices about their lives and behaviors.
As a Community Health and Wellbeing Worker (CHWW), you will understand and work within your local community. The outreach work of a CHWW is essential in addressing the rising inequities in health and social care and in identifying unmet needs within your community. The CHWW will help provide a bridge between community, health, and care, as well as support people in accessing the right services for them.
The CHWW will need to build long-term trusted relationships with a range of householders and develop consistent relationships with the aim of improving the health and wellbeing of each individual you work with. You will possess a level of self-confidence and personal robustness to enable you to work with the community, helping people overcome their barriers through a coaching approach. Insight into the challenges that people can face, which may come from personal lived experience, volunteering, or other employment, is essential. You will maintain a culture of inclusivity and learn to work in a trauma-informed way.
This role offers the opportunity to collaborate with organizations across the voluntary and health sectors, focusing on what matters to people and empowering them to find local solutions while shaping a new role for Cornwall and the Isles of Scilly.
