Job Overview
The post holder will be part of a dynamic team of clinicians responsible for developing and improving services for people over 18 years with common mental health problems in primary care. The role includes working across designated GP practices within the primary care networks and attending weekly integrated care team meetings.
The post holder will provide effective consultation and liaison across the primary and secondary care sectors, facilitating and improving access to appropriate services. Responsibilities include mental health screening assessments, risk assessment, medication monitoring, telephone triage, signposting, referral to other services, and brief intervention for individuals experiencing common mental health problems.
The post holder will also provide support, education, and problem-solving advice to other members of the local primary care teams, particularly in the areas of assessment and management of mental health problems, working across different geographical settings and undertaking home visits.
Main Duties of the Job
* Undertake specific managerial, administrative, preceptor, and clinical supervisor roles as required.
* Ensure up-to-date, accurate clinical notes and activity recording relevant to the role.
* Participate in audit and appropriate approved research.
* Actively involve in developing services to meet the mental health needs of users and carers in primary care, ensuring they are equitable, non-stigmatising, and responsive to local need.
* Maintain and improve knowledge and skills based on best practices, particularly in mental health issues in primary care.
* Act as a role model and resource for junior/less experienced staff and students from the multi-disciplinary team.

Detailed Job Description and Main Responsibilities
* Provide triage and assessments of mental health needs using sound knowledge and evidence-based outcome measures, including risk assessment and risk management.
* Work in partnership with service users to identify effective strategies to promote understanding and management of presenting mental health problems.
* Provide information, advice, education, health promotion, and liaison for referrers, service users, carers, colleagues, and the wider community about all aspects of mental health and available options.
* Critically analyse service user information and formulate individual care plans using evidence-based brief interventions.
* Signpost and support referral of service users to a range of services, including primary, secondary, and voluntary agencies, and follow up as required.
* Participate in the development and delivery of primary care-based therapeutic and support groups in response to local need.
* Attend and actively participate in multidisciplinary integrated care team meetings weekly in surgeries across the borough.
* Participate in appropriate managerial, clinical, and caseload supervision in accordance with Trust policy to support all clinical work undertaken.
* Facilitate the stepped care model/clinical pathways for clients experiencing enduring mental health problems.

* 12 months experience working as a qualified nurse in mental health
Desirable Criteria
* 6 months community experience in mental health
