Provide practical help, support and referrals to partner agencies for S/U's who need help with managing their home and tenancy and encouraging S/U's to access specialist advice if needed.

Main duties of the job
To provide support, give time to an allocated group of service users and thuspromote their recovery and promote their place in the broader community.o To assist the care co-ordinator or key worker to assess, plan, implement andevaluate individual service user lead support / recovery plans focusing on providingsettled accommodation solutions, in particular social care needs assessment andeligibility for a personal budget.o To contribute towards the management of a team of STR workers, by leading onclearly identified, appropriate day to day team management tasks.o Responsible for providing support to the care co-ordinator, in the implementationof a recovery focussed care plan for an allocated number of individual serviceusers.o To ensure that service users engage beneficially with the agreed Care Plan andaccess appropriate services provided on a regular and consistent basis.o To positively promote independent living of service users within community.o To enable the capacity of the team to develop a rapport with service users, basedupon attentiveness, sensitivity, understanding, compassion and honesty.o To develop plans that will enable the provision of practical support to service usersand their carers in developing and managing dignity and independence.
