Position Summary:
We are seeking a highly motivated and experienced Director of Nursing to lead our home health care team. The ideal candidate will oversee nursing services, ensure regulatory compliance, and maintain the highest quality of patient care. This role involves strategic planning, staff management, and fostering strong relationships with patients and their families.
As Director of Nursing, you supervise the clinical operations of your team of field staff and report directly to the Agency Administrator. You will need to be organized, detail-oriented, and possess effective communication skills both orally and in writing. You will need to understand the issues related to the delivery of home health care services and be able to problem solve. Previous home health and management experience will serve you well in this rewarding role, and an excellent knowledge of OASIS and Medicare guidelines is most helpful as you will be reviewing plans of care on a daily basis.
Key Responsibilities:
Leadership and Management:
1. Overseeing the overall clinical operations of the home health agency.
2. Supervising and supporting nursing staff, caregivers, and other healthcare professionals.
3. Leading team meetings to discuss patient care, staff concerns, and operational improvements.
4. Recruiting, training, and mentoring nursing staff to maintain high-quality care standards.
Patient Care Oversight:
1. Ensuring patient care plans are created, implemented, and regularly reviewed.
2. Monitoring the quality of care provided to patients and addressing any concerns or issues.
3. Conducting patient assessments as needed to develop or update care plans.
4. Ensuring compliance with medical protocols and standards of care.
Compliance and Regulatory Tasks:
1. Ensuring the agency adheres to federal, state, and local regulations, including Medicare/Medicaid requirements.
2. QA review.
3. Preparing for and managing audits, inspections, and certifications.
4. Maintaining accurate and up-to-date clinical records for compliance purposes.
Communication and Collaboration:
1. Acting as a liaison between patients, families, and medical staff to ensure seamless communication.
2. Coordinating with physicians, therapists, and other healthcare providers for comprehensive patient care.
3. Reporting to senior management about operational updates, challenges, and achievements.
4. Participating in sales and marketing initiatives.
Problem-Solving and Decision-Making:
1. Addressing staff performance issues and implementing corrective actions when necessary.
2. Resolving conflicts or challenges related to patient care or staff concerns.
3. Developing and implementing strategies to improve operational efficiency and patient satisfaction.
Qualifications:
Education and Licensure:
1. Active Registered Nurse (RN) license in the State of Arizona.
Professional Experience:
1. Minimum of 3 years of nursing experience with 2 years in Home Health, with a minimum of 1 year in a leadership or supervisory role.
2. Experience in home health care, hospice, or community health is highly preferred.
3. Familiarity with case management and coordinating interdisciplinary care plans.
Skills and Competencies:
1. Strong leadership and management skills to oversee staff and clinical operations.
2. Home Health coding experience preferred.
3. Excellent knowledge of Oasis.
4. Proficiency in home health regulations, including Medicare/Medicaid compliance and quality standards.
5. Excellent communication and interpersonal skills to collaborate with staff, patients, families, and other healthcare providers.
6. Analytical abilities for problem-solving and making informed decisions in complex situations.
7. Organizational skills for handling multiple responsibilities, from patient care oversight to administrative duties.
8. Responsible for the QA/PI activities.
Technology Proficiency:
1. Comfortable using electronic medical records (EMR) systems and other healthcare management software.
2. Basic knowledge of Microsoft Office Suite (e.g., Word, Excel, Outlook) for documentation and communication.
Additional Requirements:
1. CPR certification.
2. Valid driver’s license and auto insurance.
