The service is looking for multiple health care support workers.
Band 3 Health Care Support Worker work closely with people who use our Services, their families, friends and carers and play an important contribution in how people experience our services. Band 3 Health Care Support Workers are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by:
• keeping the people who using our services as safe as possible by recognising the limits of the Health Care Support Worker role and working closely with Registered Nurses
• contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures when implementing care plans
• ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans
Band 3 HealthCare Support Workers contribute to the team achieving its objectives by working closely with and drawing support from Registered Nurses and other health professionals.
All interviews are conducted in a face to face setting.
Band 3 Health Care Support Workers are expected to contribute to the effectiveness of services by:
• Providing skilled, evidence based care which adheres to agreed policies and procedures
• Working with patients and families and registered nurses to contribute to all stages of the care planning process including assessing risks and needs
• Acting as patient advocates in the multi-disciplinary team
• Working as part of a team implementing care plans or directives overseen by registered nurses
• Contributing to creating and maintaining high performing teams by:
• communicating well with all members of the team
• understanding their role in the team and how they help the team achieve its’ objectives
• reflecting on their own practice regularly
• identifying how care could be improved

Come and join the Home1st inpatient intermediate care team at Windsor Intermediate Care Unit, 19-bedded unit in Bletchley. Our purpose is to provide an enhanced Intermediate Care, rehabilitation and sub-acute service predominately for the older person. Preventing acute hospital admissions and facilitating early discharge are our primary goals.
This post offers the opportunity to further develop your knowledge and skills in a nurse led environment. You should have a genuine interest in the care of the older person and rehabilitation.
You will need excellent communication skills and a desire to help develop practice and services that make a real difference to service users.
Supported by our Advanced Care Practitioner and registered nurses, your development needs will be regularly reviewed and you will be encouraged to learn new skills and build upon your existing knowledge.
• To facilitate and support patients communication throughout daily activities
• To assist and run individual or group activities as required
• Assist patients to gain adequate nutrition by serving meals/drinks taking into consideration dietary/nutritional requirements and helping with eating and drinking as needed
• To assist patients carrying out exercise programs relating to mobility, communication, feeding and drinking as directed by members of the multi-disciplinary team
• Assist patient in transfer practice and mobilization
• To assist members of the multi-disciplinary team in discharge planning and assessments for equipment where appropriate
• To take telephone messages accurately and relay messages to appropriate personnel
• To comply with health and safety policies, reporting accidents/incidents, ‘near misses’ or hazards in accordance with agreed local procedures and CHS policies.
