The role involves supporting staff to access care within the spectrum of community environments in school, home and respite environments. Main duties of the job Our wider service is extensive and varied and can offer opportunities to work with children who are technology dependant, children's palliative care, respite care for medically complex children, children's continuing care and medically complex children in our special school nursing. As a CCN, you would be placed primarily within a specific team but will also support between the teams in the wider service. The post holder would be expected to work independently to assess patients, plan and implement appropriate models of care, under the guidance of the band 6 nurses. The post holder will provide clinical care to children and young people with complex physical health needs in a variety of community settings, primarily patients' homes, the respite unit and special schools, but may include hospitals and hospices. They will also provide training and support to unqualified staff, using a Competency Framework. Team working and especially experience in multi-agency working is essential as the post will also involve working with education and social care to provide holistic care to CYP on the caseload. The successful candidate will have good communication skills, be a team player whilst being able to work autonomously when required. A sense of fun and creativity are also valuable traits.
