Our client is a well-established charity that provides many services to the local community of Wakefield and the surrounding areas. These services support all ages, and the charity is well respected locally.
They now need a community wellbeing coach to join their team and support the integrated neighbourhood team by providing support to local people who require encouragement, coaching, and motivation to pick life back up after an illness or a life event and reintegrate into social situations and events.
The requirements could stem from a variety of situations that have resulted in a local person feeling isolated. Tasks may include meeting for a cup of tea and encouraging them to be out and about more, through to encouraging and enabling someone to go out after becoming blind, helping them to use public transport and accompany them until they are confident to do this alone. There are many people who access the charity for community support and guidance. This role is to assess each person and conduct one to a few visits before passing them to a volunteer who will then keep in touch. A record of notes from each visit will be entered into the data system for safety.
Responsibilities
* Support the delivery of the Connecting Communities within West Wakefield.
* Work with individuals to help them create goals and aspirations that support health and wellbeing through a non-medicalised approach.
* Work with the Volunteer Co-ordinator to recruit support volunteers and befrienders, matching them with individuals supported by the project.
* Provide information and records of visits to the Engagement and Partnerships Service Manager to produce reports for senior managers and commissioners.
* Help raise awareness of the project within the community.
* Gather initial client information and ensure the referral meets the guidelines set within the project.
* Direct and oversee befriending volunteers and inform them of each client’s needs that they will be looking after.
