Locala Community Rehabilitation Service provides comprehensive high quality therapy care for adults (anyone over 16) with multiple conditions, frailty, those who have fallen, or with Neurological conditions. An exciting opportunity has arisen for a rehabilitation assistant to join our team. We have teams working from Mill Hill Community Health Centre in Huddersfield and Eddercliffe Health Centre in North Kirklees, both of which have good motorway links. The Service provides high quality care to patients in their own homes, community rehabilitation facilities and care homes. This role requires a motivated and dynamic person to work within this established service and it is essential that you have experience of working as part of a multidisciplinary team as you will be working with Occupational Therapists, Physiotherapists, Dietitians, Speech and Language Therapists, assistant practitioners and other rehabilitation assistants to provide follow up rehabilitation programmes and support for patients. Good communication skills and flexibility are essential qualities and a commitment to working with patients to improve their quality of life. Experience of working in a health or social care setting is required as well as an awareness of the role of rehabilitation. As an organization we use electronic records, so IT skills are desirable, as are an ability to prioritise and demonstrate good organisational skills.
