Older Adult Home Treatment Team (OAHTT)
Hours: 37.5 per week
Salary: Band 5 £28407 - £34581 pa (dependant on experience)
We are looking for enthusiastic and motivated registered mental health nurses to join our Older Adult Home Treatment Team. The Older Adult Home Treatment Team provides a service 8-8pm, 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home.
You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of a wider Multidisciplinary team.
Main duties of the job
The post holder will have continuing responsibility for the provision of skilled intervention for people assessed and diagnosed with significant mental health problems in the community.
The post holder will work within a multi-disciplinary service under the supervision of experienced staff, and core responsibilities will include:
* Acting as a Care Co-ordinator/Lead Professional.
* Work within Care Programme Approach.
* Carrying a varied caseload defined by need.
* Contribute to a duty system on a rota basis.
* Contribute to multi-disciplinary service development.
The post holder will:
* Value and be responsive to the individual rights, needs and opinions of the people using our services.
* Be responsible for the ongoing assessment, planning, implementation and evaluation of programmes of nursing care to an agreed standard.
* Undertake initial nursing assessments, care planning and risk assessments as agreed.
* Participate in monitoring the standard of care delivery by Support Worker staff and the education of colleagues, learners.
* Maintain and promote a patient centred approach in all aspects of service delivery.

Job responsibilities
Clinical
* Work across a variety of community settings as part of a multi-disciplinary team.
* Provide nursing assessments and identify clients' mental health needs at home/in their own environment.
* Formulate, implement and evaluate collaborative care plans that reflect the needs of clients/carers.
* Manage own caseload with a degree of autonomy until point of discharge.
* Administer depot injections and monitor the effects of prescribed medication.
* Maintain accurate care records according to Professional and Trust standards.
* Act as co-ordinator in accordance with the Care Programme Approach.
* Deliver a range of therapeutic interventions in accordance with evidence-based practice.
* React to and manage a range of complex mental and physical health crises.
* Escort clients where appropriate to other Health and Social Care settings.
* Offer support and advice to families and carers.
* Identify and manage risks and document them in DRAMs and risk reduction plans.
Supervision
* Manage a caseload with appropriate levels of supervision.
* Provide direct supervision to junior staff and assist with the PDR system.
* Liaise with and educate other professionals involved in the care of older people with mental health needs.
* Act as identified supervisor/mentor for nursing students on allocated placements.
* Participate in maintaining and developing high standards of professional and clinical practice.
* Share identified skills with colleagues and participate in teaching.
* Take responsibility for identifying personal development needs.
* Act as a positive role model.
* Offer education and health promotion to clients and carers.

Person Specification
Training
* Evidence of current NMC registration.
* Experience of working in a community setting.
* Experience of working with Older Adults in a mental health setting.
Knowledge and Skills
* Ability to respond to the individual rights/needs/opinions of service users.
* Good risk assessment and management skills.
* Knowledge of the mental health needs of older people.
* Excellent written and verbal communication skills.
* Skills in care planning and evaluation of individual plans of care.
* Knowledge of the MHA/MCA and relevant legislation.
* Basic IT skills.
* Evidence of professional development.
Experience
* Full driving licence and access to a vehicle.
* Relevant post-qualifying experience in mental health nursing.
