Job summary
We are seeking a general practice nurse to join our 3300 patient list size.
Full time position. Ideally Mon- Fri
7.30 am- 6.30 pm as required by the business. Friday close 1pm. No weekends unless planned immunisation clinic.
Applicants should be registered with NMC and have experience working within a primary care setting. The applicant should be a team player, highly motivated, and organised.
Main duties of the job
To oversee all practice nurse duties in this practice:
1. Respiratory disease management (COPD and Asthma)
2. Chronic disease management clinics (hypertension, diabetes, CHD, CKD)
3. Perform routine tasks related to patient care as directed by GP
4. Tissue Viability wound and ulcer care, doppler assessments and compression
5. Removal of Sutures
6. ECGs
7. Venepuncture
8. Travel Health
9. Adult and childhood immunisations to include delivering the seasonal flu campaign
10. Contraception injections and routine checks
11. Cervical Cytology
12. Chaperoning and assisting patients where appropriate who are being examined by another clinician
13. Request pathology tests, for example, urine culture, swabs
14. Contact patients by email and telephone to offer advice and support
15. Be competent in the use of Emis web clinical IT systems and all other IT systems used as part of the Practice Nurse role.
16. Maintain general tidiness and cleanliness of nurses and treatment rooms, including stocking and rotating items as required
17. Maintaining and cleaning equipment used by nurses and GPs
18. Participation in administrative systems in the practice
19. At all times there will be a need to maintain accurate records
20. Attend and participate in Practice and team meetings
21. On occasion there may be a requirement to undertake home visits
22. Maintain a sound knowledge of the GMS contract for chronic disease management
23. Have a sound knowledge of the Practices infection control policy
24. Support in primary care offer and IIF, QOF targets

Job description Job responsibilities
The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. The post holder is accountable for nursing service delivery. They will be working closely with the practice management and GP to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients and support the delivery of policy and procedures.

Qualifications Essential
* Must be a registered nurse
* Community nursing specialist qualification or equivalent
* Skills in management of patients with long-term conditions including COPD and Diabetes
* Awareness of accountability of own and others roles in a nurse-led service
* Knowledge of health promotion strategies
* Awareness of local & national policies relating to health
* Knowledge of clinical governance issues in primary care
* Knowledge of patient group directions and associated policy
* Leadership skills
Desirable
* Clinical skills in cervical cytology, immunisation and vaccination, ear care, COPD and diabetes, phlebotomy
