Job summary Community Nurse Practitioner Permanent -37.5 Hours We are seeking to recruit a Community Nurse Practitioner to Join the Community Rapid response team withing Dudley clinical Hub to provide and support the provision of high-quality patient care. This is an exciting opportunity for an experienced, dynamic, enthusiastic, and motivated practitioner to develop new ways of working. They will be integral in delivering community services in different ways across the localities. As a Community Nurse Practitioner, you will be responsible for the assessment of the health and clinical needs of the patient, to undertake consultations with patients as an autonomous practitioner using advanced clinical practice skills and own clinical judgement to diagnose, treat, to prescribe where required, refer to other services and discharge patients. Main duties of the job A key part of the role will include making rapid clinical assessments of patients with undifferentiated conditions, making evidenced based treatment plans whilst aiming to ensure patients are cared for in the most appropriate place. As part of the Urgent Community Response Team, the practitioner will be expected to respond to patients who are experiencing crisis within a 2 hour national target. You will be required to ensure delivery of high quality patient centered care measured by clinical effectiveness, patient safety and patient experience as well as meeting the key performance measures as required by the Trust. This is an exciting role for the right candidate. You will be joining a developing and progressive service, giving you the opportunity to participate in the ongoing development of the team and the Clinical Hub. The successful candidates must have experience of service development and clinical leadership to ensure patients and their families receive timely, effective and supportive care. You will be managed and supported daily by the Advanced Clinical Practitioners, Hub Lead and Medical Lead as well as giving support to junior staff within the Dudley Clinical Hub.
